In the event of a potential MATERNITY CLAIM
In order to make a claim for Maternity reimbursement:
Complete a VanCare reimbursement claim form and include;
- Details such as the Full Name of the Insured, claimant or dependent whichever is applicable with their VNPF number and date of birth
- Submit original receipts and invoices to VanCare.
- Breakdown on receipts, diagnosis should be shown on the claim form.
- On receipt of the above, then VanCare will process the claim further.
Note: Please ensure that your premium is kept current and up to date, as we cannot process any claim where the premium is in arrears.
