In order to make a claim for TPD:

• Obtain a medical report from the attending doctor and submit it to our office.
• Complete a medical attendant's statement form. (by the attending doctor)
• Submit a letter from employer confirming Insured is disabled and cannot carry out his/her normal duties (if Insured is employed).
• Submission of the following documents;
  
  • - Original Birth Certificate
  • - Complete VanCare MPI Claim Form
  • - Statement of Account
• On receipt of the above, then VanCare will process the claim further.

Note: Please ensure that your premium is kept current and up to date, as we cannot process any claim where the premium is in arrears.
