In order to make a claim for Term Life:

Complete a VanCare Death claim application form and include;

• Details such as the Full Name of the Insured, claimant or dependent whichever is applicable with their VNPF number and date of birth, Occupation, address, duration of illness, treating doctor, details of hospital treatment over the last 12 months
• Submit original birth certificate of the Insured.
• Submit original death certificate of the Insured.
• On receipt of the above, then VanCare will process the claim further.

Note: Please ensure that your premium is kept current and up to date, as we cannot process any claim where the premium is in arrears.
